I, the undersigned parent/guardian, hereby give permission for my child/ward to participate in the
Peekok YouthARTS Performing Arts Program. I have read, understood, and agree to the terms and
conditions of the program, including but not limited to:
1. Full participation in rehearsals and scheduled performances.
2. Adherence to program rules, behavior expectations, and safety guidelines.
3. Non-refundable registration fee of 100,000 NGN paid by August 30, 2025.
4. Consent for use of photos/videos of my child/ward for promotional purposes.
5. Acknowledgement that international travel opportunities are selective and may require additional documentation and expenses.
6. Disclosure of any relevant medical conditions or special needs. I understand that Peekok YouthARTS will make every effort to ensure a safe and enriching experience, but will not be held liable for unforeseen incidents.
